My wife and my very first girlfriend (we both lost our virginity to each other.) Three reasons:
1. My wife can't get TOO jealous...I've already had sex with this girl many times. (Well, maybe that's reason to get even more jealous than if she was a stranger. I don't know, who cares.)
2. She's still plenty good looking.

If it's FFM you really need to have one girl who not only is freaky as heck, but is borderline bi and has no problem getting down with/on another girl - otherwise you end up with dead time for one of the girls at various points, and that just kills the fun for everyone.
